  2. If you have Medicaid Long-Term Care, ask your case manager to send the referral to Salus Home Health.

  3. Our registered nurse visits your home, reviews your needs and creates your plan of care with you.

  4. We match you with a trained home health aide and services begin.

What to expect

A registered nurse supervises your care with regular home visits. Your aide follows your written plan of care, and you can call our office anytime with questions or changes.

You have the right to be treated with dignity and respect, to take part in planning your care, to privacy of your information, and to voice complaints without fear of losing your services.
